History of Eliquis

Apixaban was discovered by researchers at Bristol-Myers Squibb. The compound is a small molecule that selectively inhibits factor Xa, an enzyme in the coagulation cascade that plays a central role in blood clot formation. By blocking factor Xa, apixaban reduces the production of thrombin, the enzyme that converts fibrinogen to fibrin, thereby preventing clot formation.

BMS and Pfizer announced their worldwide collaboration on apixaban on April 26, 2007. The agreement covered development and commercialization across a broad range of venous and arterial thrombotic conditions. At the time, apixaban was in Phase III clinical trials. The European Medicines Agency approved Eliquis in May 2011 for prevention of venous thromboembolism after elective hip or knee replacement surgery. The FDA approved Eliquis on December 28, 2012, for reducing the risk of stroke and systemic embolism in patients with nonvalvular atrial fibrillation.

Subsequent FDA approvals expanded Eliquis's indications. In 2014, the FDA approved Eliquis for the prophylaxis of deep vein thrombosis and pulmonary embolism in patients who have undergone hip or knee replacement surgery. Later that year, the agency approved Eliquis for the treatment of DVT and PE, and for reducing the risk of recurrence of DVT and PE following initial therapy.

The ARISTOTLE trial, published in the New England Journal of Medicine in 2011, was the pivotal study that demonstrated Eliquis's superiority over warfarin. The trial enrolled over 18,000 patients with atrial fibrillation and showed that apixaban reduced the risk of stroke or systemic embolism by 21 percent compared to warfarin, while also causing significantly less major bleeding and lowering overall mortality. These results established Eliquis as a preferred oral anticoagulant and drove rapid adoption.

By the mid-2010s, Eliquis had become the market-leading oral anticoagulant in the United States and globally. Sales grew from approximately $774 million in 2015 to over $10 billion by 2022. In 2025, worldwide Eliquis revenue reached $14.4 billion, with BMS reporting $10.2 billion in U.S. sales and $4.2 billion internationally, and Pfizer reporting approximately $7.96 billion in combined alliance and product revenues.

In 2026, Eliquis continued to grow despite the implementation of Medicare price negotiation under the Inflation Reduction Act. A Medicare maximum fair price of $231 per 30-day supply took effect on January 1, 2026. BMS and Pfizer also began selling Eliquis directly to cash-paying U.S. patients at a discount of more than 40 percent, and reduced the list price for commercial patients. In the second quarter of 2026, BMS reported Eliquis sales of $4.48 billion, up 22 percent year over year, driven by growth in demand. BMS raised its full-year 2026 Eliquis revenue growth guidance to 20 to 25 percent, up from its previous forecast of 10 to 15 percent.

Eliquis Recalls & Controversies

Eliquis has maintained a strong safety profile since its FDA approval in 2012, but the drug has been the subject of several notable controversies and legal proceedings.

Patent Litigation: BMS and Pfizer have actively enforced the Eliquis patent estate through Hatch-Waxman litigation against multiple generic manufacturers seeking to market apixaban in the United States. In September 2024, the companies sued Indian generic manufacturer Umedica Laboratories in the District of Delaware over patent US9326945B2. The case was voluntarily dismissed without prejudice after 15 days. Settlements with other generic manufacturers established the April 2028 U.S. generic entry date.
